How are sleeves. My arms are 14 inches in diameter and I sometimes find the sleeves to be tight, which I do not like. I am thinking about ordering a size 16W

Can the dress be altered (made smaller) on the sides?

Yes. I took mine to a tailor and she is adding two vertical darts to the back to help it fit my smaller waist.
